Examinations & Screenings
This serves as notice to the parents/guardians of kindergarten, 2nd, 6th, and 9th grade students that each student must have a dental examination and submit proof of such an examination by May 15 of the school year. The dental examination must have taken place within 18 months prior to May 15 of the school year.
This requirement can be waived for students who show an undue burden or a lack of access to a dentist. The waiver form must be submitted by May 15 of the school year.
Eye Examinations & Vision Screenings
This serves as notice to the parents/guardians of kindergarten students or students enrolling in school for the first time that each student must have an eye exam by October 15. The eye exam must be completed within 1 year prior to the first day of the school year the student enters kindergarten or school for the first time in Illinois.
This requirement can be waived for students who show an undue burden or a lack of access to an optometrist or to a physician who performs eye examinations. The Illinois Department of Public Health wavier form is available at the school district or school office or for download below. The waiver form must be submitted by October 15 of the school year.
* Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo a vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months.
